About Clarity Pediatrics
Clarity Pediatrics is building a new, virtual, evidence-based model for pediatric chronic
care, beginning with ADHD. Our mission is to expand into additional chronic
conditions—including asthma, allergies, and obesity—and deliver affordable,
science-backed care to the 1 in 3 children living with chronic conditions.


Position Summary
The Community Health Worker (CHW) is a key member of the interdisciplinary care
team, working closely with physicians, advanced practice providers, psychologists, and
therapists. Under the direction of the CHW Supervisor, the CHW supports children with
chronic conditions and their families by improving access to care, addressing social
determinants of health, and coordinating resources.


Key Responsibilities
Patient Enrollment & Support
● Assist families with enrollment in Clarity Pediatrics
● Support appointment scheduling, reminders, and follow-ups
● Facilitate referrals to external clinical services when needed


Screening, Education & Resource Navigation
● Conduct Social Determinants of Health (SDOH) screenings
● Connect families to community resources
● Respond to patient inquiries via messaging
● Provide education to families about chronic conditions


Documentation & Team Collaboration
● Maintain accurate and timely patient records and reports
● Complete progress note documentation
● Participate in interdisciplinary case conferences and team meetings
● Report patient progress to Supervisors and adjust interventions as needed
● Identify workflow inefficiencies and contribute to solutions

Qualifications
Required:
● Community Health Worker Certification
● Ability to learn and use multiple technology platforms
● Strong organizational, communication, and interpersonal skills
● Ability to manage multiple priorities and work collaboratively
● Comfort working with diverse populations


Preferred:
● Bilingual English/Spanish
● Experience working with socially and medically complex patients
● Experience in underserved or urban environments
● Familiarity with San Francisco Bay Area community resources


Logistics
● Full-time, contract position
● Initial 3-month contract with potential extension
● $25/hour
● No weekend or on-call responsibilities
● Hybrid (Office/Remote)
● Valid driver’s license and insured vehicle required
