Role Description
The Community Supports Team Lead will join Zócalo Health at a critical time in the history of our company and of primary care. The opportunity for impact is large and growing, as Zócalo Health leads the transition from fee-for-service to value-based care for the growing Latino community in the U.S.
As a member of the Zócalo Health team, you will be responsible for overseeing the Housing Services programs, ensuring operational excellence, quality of care, and team development. You will provide leadership, coaching, and supervision to the Housing Navigator team, drive process improvements, and support member and partner escalations to ensure members achieve housing stability and related care goals.
This position reports to the Senior Manager, Community Supports.
The Community Supports Team lead will contribute in the following ways:
- Provide direct supervision and coaching to Housing Navigators, including weekly 1:1s, performance support, and professional development.
- Lead operational oversight, monitoring team workflows, member engagement, caseload distribution, and timely documentation in Fold, Athena, and HMIS.
- Conduct quality assurance, including chart audits, feedback delivery, and ensuring compliance with MCP, CalAIM, and Zócalo Health standards.
- Handle complex member or partner escalations, supporting Navigators with housing placements, landlord conflicts, or service coordination.
- Lead process improvement initiatives, updating SOPs, templates, workflows, and training materials to ensure program consistency and clarity.
- Collaborate across functions to integrate services and improve member outcomes.
Qualifications
- Minimum 3–5 years of experience in housing navigation, case management, or social services, with demonstrated leadership experience.
- Strong knowledge of housing programs, tenancy support, and related social services.
- Experience supervising teams, providing coaching, and supporting professional development.
- Familiarity with electronic health record systems and data management tools (e.g., Athena, Fold, HMIS).
Preferred Qualifications
- Experience in managed care or value-based care environments.
- Fluency in Spanish and experience serving Latino communities.
- Experience leading process improvement initiatives and quality assurance in health or social service programs.
